Physical TherapistSTARTING RATE: $33.00/hr. ($33.00 - $37.00/hr., dependent upon experience)SHIFT: FT DaysBENEFITS:Weekly PaychecksReferral BonusesPeer MentorshipJob Summary: The Physical Therapist will evaluate all patients/residents admitted to the facility and will recommend, in writing, to the attending physician a rehabilitation program if indicated. Upon receipt of the physicians' orders, the Physical Therapist will be responsible for the interpretation and implementation of service and programs concerning physical therapy. The physical therapist will, on a regular basis, review and evaluate all patients/residents, be responsible for all documentation of patients/residents' care plans. Attends and actively participates in all staff and committee meetings related to improving patient /resident care and maintaining the physical therapy department.Job Performance Responsibilities:Evaluates all patients/residents admitted to the facility within 24 hoursRecommends in writing to the attending physician a rehabilitation program, if indicated, in their professional's judgement, to include but not be limited to restorative nursingUpon receipt of physicians' orders interpret and implement those services and programs related to physical therapy.Ensures that orders are carried out in a professional manner, satisfying professionals ethics, patients/residents rights, while maintaining State and Federal Health department codesConducts initial patient/resident evaluationCreates short- and long-term goalsAssesses rehabilitation potentialDevelops and implements a treatment plan which may be delegated to a PTA if applicableEngages in professional information communicationDocuments all patient/resident charts and nursing care plansEnsures that proper equipment order for home discharged and HEP given as appropriateAssigns appropriate therapeutic prescription of minutes while monitoring accordinglyCompetent in long term care, sub-acute, and outpatient therapiesProvides appropriate patient educationJob Qualifications:Bachelor degree of Physical Therapy2-3 years' experience in long term careLicensed and registered in NYSABOUT US:VestraCare is a portfolio of four health care facilities in New York state, including: Chautauqua Nursing and Rehabilitation Center in Dunkirk; Roscoe Rehabilitation and Nursing Center in Roscoe; Sunset Nursing and Rehabilitation Center in Boonville; and Susquehanna Nursing and Rehabilitation Center in Johnson City.
